To sign up with Home Care Heroes as an agency managed Member, you will need:

- Your NDIS plan number

- The NDIS plan start date and end date

- You will need to know the funding area you want to use (such as core supports or capacity building)

- You will need to know the amount of funding you would like to allocate to Home Care Heroes.

Are you Plan-Managed?

To sign up with Home Care Heroes as a plan-managed Member, you will need:

- Your NDIS plan number

- The NDIS plan start date and end date

- You will need to know the funding area you want to use (such as core supports or capacity building)

- You will need to know the amount of funding you would like to allocate to Home Care Heroes.

- The company name, contact phone number and email address of your plan provider.

Are you Self-Managed?

To sign up with Home Care Heroes as a self-managed Member, you will need:

- Your NDIS plan number

- You need to know the funding area you want to use (such as core supports or capacity building)

- The email address you would like all invoices sent to
